Loose Neck Skin & Double Chin
Loose Neck Skin & Double Chin
"I'm so embarrased of neck." A fullness under the chin, bands down the front of the neck, or skin that has started to hang, and scarves in summer. For many people the neck ages before the face, and it does so in different ways. A double chin survives every diet. Two vertical cords show when you talk. Sometimes the skin under the chin has simply loosened and hangs. Most people notice it in a side-on photograph, or in the way a collar sits. Because the causes differ, so do the answers, and picking the wrong one is how people end up disappointed.

WHAT CAUSES IT
Why it happens
Three things make a neck look older or fuller, and they are treated differently. This is why the neck is examined before any treatment is chosen. The same look can come from different causes, and the right treatment follows the cause.
01
Fat under the chin
A pocket of fat between the chin and the neck that blunts the angle between them. It runs in families and it does not respond to weight loss the way the rest of the body does.
02
Muscle bands
The platysma muscle has separated in the midline, so two cords appear down the front of the neck, worse when you speak or grimace.
03
Loose skin
The skin has lost its elasticity and hangs under the chin, sometimes over a neck that is otherwise fine, sometimes with the other two.
04
A small chin
A chin that sits back makes the neck look full and the angle look blunt when neither fat nor skin is the real issue.
WHAT ACTUALLY TREATS IT
What actually treats a loose neck and double chin in Barrie
Several different things make a neck look older or fuller, and each one is treated differently. Some of the answers are surgical and some are not, and both routes are available here. The consultation decides which cause is yours.

Neck Liposuction
Removal of the submental fat pad, the pocket of fat between the chin and the neck, through two or three tiny entry points under local anesthetic.
People whose double chin is fat rather than skin, in a neck whose skin still springs back. That pocket runs in families and does not respond to weight loss the way the rest of the body does.
The fat pad that blunts the angle between the chin and the neck is removed, which restores the angle. Nothing is done to the skin, which is why it suits skin that still has its elasticity.
Most patients are back at desk work in three to five days, with a soft chin strap at night. The final contour shows at three months.

Neck Lift
The full neck operation. It stitches the separated edges of the platysma muscle back together and removes the loose skin of a turkey neck.
People whose skin has loosened and hangs under the chin, with or without the two cords that show when you speak. A blunt angle that is really a chin sitting back is a different route, and that is settled at the consultation.
The muscle edges are stitched back together to deal with the bands, and the skin is tightened through incisions behind the ears. Where the face has gone with the neck, a face lift and neck lift are done together.
Ten days to two weeks before desk work. The result settles over three months and typically holds for ten years or more.

Mini Neck Lift
A shorter version of the neck lift, done through a single hidden incision under the chin.
People with platysma bands and modest laxity, where the skin does not need removing.
The muscle edges are stitched back together through the one incision under the chin. Because no skin is taken away, the incisions behind the ears are not needed.
About a week before desk work. The timeline for your case is confirmed at the consultation.

Fat-Dissolving Injections
Belkyra, a deoxycholic acid injection that dissolves submental fat over a series of sessions. Dr. Tumi performs all injectables himself.
People with a smaller pocket of fat under the chin who want it treated without surgery.
The injected acid dissolves fat in the pocket under the chin. It is given over a series rather than in a single visit.
Two to four sessions, a month apart, with the result building over that series.

CoolSculpting
A submental applicator that freezes the fat pad under the chin. It is delivered by the clinical team under Dr. Tumi's direction.
People with a double chin who want no needles and no downtime.
The applicator freezes the fat in the pocket under the chin, over a course of two to three sessions.
Two to three sessions. The timeline for your case is confirmed at the consultation.

Morpheus8
Radiofrequency microneedling used to tighten mild neck laxity and crepey neck skin. It is delivered by the clinical team under Dr. Tumi's direction.
People with mild laxity or crepey skin on the neck. It is not a fix for a true turkey neck, where the skin needs removing.
Radiofrequency is delivered into the skin through microneedles to tighten it. It works on the skin and does not tighten the platysma muscle or take skin away.
The number of sessions and the timeline for your case are confirmed at the consultation.

What to expect
After neck liposuction, most patients are back at desk work in three to five days, with a soft chin strap worn at night. The final contour shows at three months. A mini neck lift takes about a week before desk work. A full neck lift takes ten days to two weeks. That result settles over three months and typically holds for ten years or more. Belkyra shows its result over two to four sessions a month apart. The timeline for your case is confirmed at the consultation. See before and after results

Who provides your care
Surgeon-led care you can verify
You are assessed by Dr. Kamal Tumi, M.D., F.R.C.S.(C), a board-certified plastic surgeon with more than 28 years in practice and over 3,500 surgeries performed, and a consultant plastic surgeon at Royal Victoria Regional Health Centre since 2004. The surgeon who assesses you is the surgeon who operates. Injectable treatment is performed by Dr. Tumi himself. Non-surgical treatment is delivered by our trained clinical team under his direction, including Priscilla Carvalho, medical aesthetician, phlebotomist and skin therapist. Smoking materially affects healing for most procedures, which is why you will be asked about it. His CPSO registration is public and searchable.
“I will not do a surgery on you if you do not need it.”
An assessment and a conversation.
You do not need to arrive knowing which procedure you want. Describing what bothers you is enough. You are not committing to surgery by booking a consultation.
FAQ
About Barrie Plastic Surgery
Yes. He holds an M.D. and is certified by the Royal College of Physicians and Surgeons of Canada as a specialist in plastic surgery. That certification separates a plastic surgeon from a physician who offers cosmetic treatment. He has been a consultant plastic surgeon at Royal Victoria Regional Health Centre since 2004.
In everyday use they are used interchangeably, and both terms describe most of what we do. Strictly, plastic surgery is the recognized surgical specialty, which includes reconstructive work. Cosmetic surgery describes procedures done to change appearance. Dr. Tumi is qualified in the specialty.
It depends on the procedure. Some, including upper eyelid surgery, are done in our own office under local anesthetic. Larger procedures are booked in an operating room, and which setting applies to you is confirmed at your consultation.
Yes. Male patients are a normal part of the practice. Eyelid surgery and gynecomastia are the usual reasons, and skin procedures are common too. ---
Unit 24, 222 Mapleview Drive West in Barrie, just off Highway 400 at the Mapleview exit. Consultations, injectables and the office procedures happen here. Operations that need a general anesthetic are done at Royal Victoria Regional Health Centre, where Dr. Tumi has been a consultant plastic surgeon since 2004.
Dr. Tumi performs facial, breast and body surgery. Skin procedures are part of the practice too: mole removal, skin cancer removal, scar revision. On the non-surgical side he injects the neuromodulators and fillers himself. Laser, CoolSculpting, Morpheus8 and skin treatments are delivered by the clinical team under his direction.
Yes, for anything surgical, and for most non-surgical treatment too. The consultation is an examination and a conversation. Dr. Tumi looks at what is actually causing what you see, tells you what can and cannot be changed, and gives you a written quote afterwards. You are not asked to decide on the day.
Dr. Tumi performs every surgery and every injection himself. The surgeon who assesses you is the surgeon who operates. Laser, CoolSculpting, Morpheus8 and skin treatments are delivered by the clinic's trained clinical staff, under his direction.
Yes. Cosmetic procedures are not covered by OHIP, and we work with three financing providers: Beautifi, Medicard and Credit Medical. Every quote is given in writing at the consultation, so you know the full figure before you decide anything.
Dr. Tumi examines the area himself. He explains what he finds in plain language, including what he would not recommend, and offers the smaller option where one exists. You leave with a written quote and time to think. If nothing surgical is the honest answer, you will be told so.
